| Coverage
& Emergencies -
Physician coverage is provided 24 hours a day 7 days
a week. During routine office hours our staff will contact
the physician and have your concerns appropriately directed,
as per the physician instructions. After routine office
hours, our answering service will contact the covering
physician. |
Telephone
Calls
- All
patients are encouraged to call with questions of
concern. Please be mindful that it would be impractical
for the physician to personally answer each call.
The staff has been trained to handle many of your
questions. They will relay your information to the
physician and have your call returned during call
back hours. After routine office hours, your calls
will be forwarded by the answering service and will
be answered at the earliest opportunity. |
Fees
& Payments
- We make every effort to minimize the cost of your
health care, while maintaining the highest of quality
care standards. If you have any questions, we encourage
you to discuss them with us. Payment is expected at
the time services are rendered. There may be additional
charges for lab work, injections and other evaluations
based on your insurance coverage.
$15.00
Service Charge may be added to account balances that
are sent to collections.
$20.00
Service Charge may be added to account balances for
any returned check. - PER OCCURRENCE
There
is a $10.00 administrative fee for all CO-PAYS that
must be invoiced. Please pay your CO-PAY at the time
of your visit.

Insurance
-The responsibility of payment is a direct obligation
of the patient. Please remember that insurance is
a method of reimbursement for medical costs, not a
substitute. Some insurance companies pay fixed allowances
and others pay a percentage of the charge. It is your
responsibility to pay any deductible/copay amount,
or any balance not paid by your insurance company.
If
your insurance company sends a check to you for a
service provided by our office, it was done so by
mistake. Your submitting the check to our office at
your earliest opportunity allows us to minimize the
administrative costs of your health care. |
| Prescription
Refill Policy -
Prescription
Refills will require 24 hours notice for regular and
48 hours notice for controlled substances. This is necessary
to accommodate established and new compliance regulations.
We appreciate your cooperation and assistance. Don't
forget to give yourself enough time to get a refill
or fill a new prescription! |
Primary
Office Choice -
To provide efficiency and stability with your care
we ask that you choose ONE
preferred office for regular visits. If an emergency
develops we will provide service at any office location.

Office
Visit Tips -
In order to provide the best possible patient care,
it is necessary to monitor not only your blood pressure,
but also the number and types of drugs that you are
taking on a regular basis.
Please
remember to bring your medications with you to your
appointment. The nurse or doctor will check the medications
and update the record in your chart.
If
you are regularly taking non-prescription drugs or
supplements, it is a good idea to share that information
with your doctor. It is advisable that all medicine
bottles are brought to each visit for accurate medication
updates.
It
is advisable that all medications are written on an
index card and kept with your drivers license and
auto registration at all times. This can be life saving
in an emergency.

National Committee for Quality Assurance (NCQA) announced that Prime
Columbia Greene Medical Associates, PC (PCGMA) of Hudson, Catskill
and Valatie has received Recognition from the Physician Practice
Connections Program for using information systems to measure practicewide
